Steps YOU must take to obtain an appointment with a Root Cause doctor:

When making an appointment you will:
- Pay
- Login or Set up your AccuDoc Portal
- Complete required forms
- Send medical records and your driver’s license in your portal
After your appointment is complete

- This is the website you will log into and retrieve your card. It requires you to sign in with your email address and password. If you have forgotten your password, there is a “Reset Password” link that you can use to make a new password.
- After you log into the system, you’ll see a “Renew Card” button near a picture of your card. Click on that and it will take you to the payment window, where you can enter your credit card information to renew. (This is a separate fee from Root Cause’s appointment fee. This goes to the State of Ohio.)
- Once you have paid, you’ll be able to download or print your card out. If you’re doing this on a smartphone, you can take a screenshot of your card.
- If you have issues with your email address (you can’t log in or you don’t remember what it is), you can give us a call (513-845-4558) and we can change your email address in your account, and the links can be sent to your new email address.
- If desired, we will print and mail your new card for $25.
- If you are a veteran or indigent, you can mail your DD214 or Disability paperwork to the state to get a discount on the price of your card as well as at the dispensary.
